  1. Last Day of Service: We will continue to provide medical services until March 31, 2024.
  2. Medical Records: You have the right to request a copy of your complete medical records. Please complete the medical record release authorization form and return it to us by March 15, 2024. You can choose to receive your records electronically or in paper format. Please provide your email address for an electronic copy of your medical record.
  3. After Closure: Following March 31, 2024, you can obtain copies of your medical records by contacting following ways:

  4. Finding a New Doctor: We encourage you to begin searching for a new healthcare provider as soon as possible to ensure continuity of care. We are happy to provide you with a list of local physicians upon request.
